Why don’t people talk about how traumatic pregnancy and giving birth are?

Here’s something I heard recently: When there is a plane crash, there’s an investigation and recommendations are implemented immediately. Yet in maternity care, there have already been 1,400 recommendations in the last 30 years but nothing has been implemented. Why is this?
